Dear Building Official Tao and Fire Department Representatives,
I am writing to memorialize my recollection of the joint site inspection conducted on June 30, 2026, at approximately 10:30 AM at 1470 S. Valley Vista Drive, Second Floor Corridor.
Present during the inspection were Building Official Tao, several Fire Department personnel, and several other individuals. Although several individuals introduced themselves at the
beginning of the inspection, I do not recall their names or specific roles.
This email is intended solely to document my recollection of today's inspection while the events remain fresh in my mind.
1. Inspection
During the inspection, I did not observe any review of plans, permit drawings, blueprints, or other project documents, nor were any such documents shown to me. I had brought copies of
the plans and supporting documents relating to the concerns I had previously submitted, but they were not reviewed during the inspection.
The inspection consisted of a visual observation of the corridor, followed by a brief on-site discussion.
During the inspection, one of the Fire Department personnel requested that one of the other individuals present display an image on a mobile phone. The image was displayed only briefly.
I observed only a small enlarged portion containing what appeared to be partial text relating to something described as fire-rated. I was not able to review the complete image, determine
what the image depicted or what document it originated from, or obtain a copy. No copy of the image or the underlying document was provided to me.
During the inspection, I was asked whether I had any additional concerns. I responded that all of my concerns were identified in the Inspection Issue Ranking Summary that I had previously
submitted. I was advised that they had seen the document.
I also noted that the Unit 204 double doors remained closed throughout the inspection and were not opened or operated during the inspection.
2. Concerns Raised During the Inspection
During the inspection, I specifically raised the following concerns:
(a) Fire extinguisher access during an emergency.
I explained my concern that, if I was attempting to access the relocated fire extinguisher during an emergency while occupants of Unit 204 were simultaneously exiting, the corridor configuration
could create a potential conflict. I was advised that this would not be an issue.
During that discussion, one of the other individuals present indicated that the double doors would open inward, gestured toward one of the two door leaves, and stated that only that
door leaf would be used. I responded that, even if the doors opened inward, my concern involved a fire emergency in which occupants of Unit 204 would be moving into the corridor while
I was attempting to pass through the corridor to access the relocated fire extinguisher.
I was asked what type of emergency I was referring to, and I responded that I was referring to a fire emergency. I was advised that this would not be an issue.
I also noted that, based on my observations, doors to other office units in the building are often kept open during business hours. I asked whether the Unit 204 doors could likewise
be kept open, and one of the Fire Department personnel responded, "They have the right to do so."
(b) Simultaneous evacuation.
I also explained my concern that, if occupants of both Unit 203 and Unit 204 were evacuating simultaneously, the modified double-door configuration could create a potential conflict
at the corridor corner. I was advised that this would not be an issue.
Following these discussions, I was advised by Building Official Tao and the Fire Department personnel that the concerns I had raised did not constitute violations, that the modified
doorway and means of egress complied with applicable requirements, that the relocated fire extinguisher location was approved, and that no violations were identified.
3. Written Documentation
I asked whether any written documentation, report, or email would be provided regarding today's inspection findings. I was advised that no written report or other written documentation
would be issued because no violations were found.
I also asked whether there would be any written documentation that I could retain for my records, including for consultation with legal counsel if necessary. I was advised that no such
documentation would be available because no violations were found.
If possible, I would appreciate receiving a brief written confirmation stating that today's inspection concluded that no violations were identified.
4. Condition 25
After the inspection concluded, I asked Building Official Tao whether I would receive an explanation regarding Condition 25 of the City Council Resolution, which states:
"Existing rated corridors shall be maintained."
I was advised that I would receive a follow-up email regarding that matter. Accordingly, I will await that follow-up communication.
5. Purpose of This Email
Nothing in this email is intended to dispute or challenge the conclusions reached during today's inspection. Its sole purpose is to accurately document my recollection of the inspection
and the information communicated to me.
As I stated during today's inspection, my purpose in raising these concerns is simply to ensure that the applicable governmental agencies have reviewed the issues I previously submitted.
I appreciate the time taken by Building Official Tao and the Fire Department personnel to meet on site today.

Thank you for your response and for confirming that LA County Fire Department will be included in the upcoming inspection.
I note that the sealing of Unit 204's original side-entry corridor door is shown on Sheets D1.0 and A1.0 of the approved plans. I will be reviewing these sheets as part of my CPRA Request
to verify that all alterations shown in the approved plans are consistent with the as-built condition on site.
Regarding the upcoming inspection, I respectfully request the following:
(a) Please advise the scheduled date and time of the inspection so that I may be available on site if needed;
(b) Please confirm that the inspection will address the fire safety concerns I raised in my correspondence dated June 25, 2026, including the fire-rated corridor end wall, the double
door assembly rating, the fire extinguisher relocation, and the egress configuration; and
(c) Please provide the inspection results and findings in writing upon completion.

Thank you for your response and for confirming that you will be following up with the contractor on Monday. I also note that I missed your call earlier today. I would appreciate it if
all future communications could be directed to this email address.
I note that your response states that the sealing of Unit 204's original side-entry corridor door is shown on the approved plans. Could you please confirm the specific sheet number and
detail reference within the approved plans that shows this modification, so that I may cross-reference it with the documents I have requested under CPRA Request No. 2026-234?
Additionally, I note that your response states that you will verify that the fire-rated corridor will remain intact upon finalization of the permit. I respectfully ask that this verification
be conducted jointly with LA County Fire Department, given the outstanding fire safety concerns I have raised in prior correspondence, and that the results be provided to me in writing.

I am writing to bring to your attention additional alterations discovered at approximately 10:00 PM on June 24, 2026, upon passing through the shared corridor, which have not been addressed
in prior correspondence. I respectfully request written confirmation of whether these modifications were included in the approved permit scope.
Unit 204's original side-entry corridor door — the unit's corridor access point as shown in the original building plans (Sheet A-102, Second Floor Plan) — has been permanently sealed
and filled over.
For reference, the scope of the overall corridor alteration is as follows: an approximately 72-inch-wide double door (approximately 75 inches including door frames) has been installed
in an approximately 97-inch-wide corridor end wall, occupying nearly the entire width of the corridor end wall, leaving only approximately 9.5 to 10 inches of wall surface remaining
on the left side adjacent to Unit 203 (excluding door frame), and approximately 12 inches of wall remaining on the right side.
Additionally, the original recessed fire extinguisher cabinet, which was embedded in the corridor end wall, has been removed as a result of the demolition of that wall. A new surface-mounted
fire extinguisher cabinet has been installed on the side wall where Unit 204's original corridor entry door previously stood — the wall directly opposite Unit 203's entry door — which
has now been permanently sealed. As confirmed in prior correspondence dated June 24, 2026, Building Official Tao stated that LA County Fire Department has not yet approved the fire
extinguisher relocation, as the department would be consulted only after the sprinkler inspection occurs.
1. CONFIRMATION OF PERMIT SCOPE REQUIRED
The permanent sealing of Unit 204's original corridor entry door is a material change to the building's entry and egress configuration. I respectfully request written confirmation of
the following:
(a) Whether the sealing of Unit 204's original corridor entry door was included in the permit scope as submitted to the City;
(b) If not, whether a separate building permit was obtained for this modification;
(c) Whether this modification has been independently reviewed for compliance with egress requirements under the California Building Code; and
(d) Whether the sealing of the original entry door affects the fire egress design of the second floor as shown in the original building plans, and whether LA County Fire Department has
been notified of this change.
2. CHANGE TO UNIT 204'S LEGAL ENTRY CONFIGURATION AND CONDOMINIUM PLAN BOUNDARY
The original building plans show Unit 204's corridor access as a single door on the side wall of the shared corridor. By permanently sealing that door and installing a new approximately
75-inch-wide double door at the corridor end wall, Unit 204 has fundamentally altered its entry configuration from what was shown in the original building plans.
I respectfully request written confirmation of whether this change has been reviewed for compliance with the following:
(a) Resolution No. 2014-13, General Condition 6, which requires that plans incorporating all Conditions of Approval be submitted to the Planning Division for review during plan check;
(b) Resolution No. 2014-13, Condition 25, which requires that existing rated corridors be maintained; and
(c) Whether this alteration constitutes a change to the unit's legal boundary as defined in the recorded Condominium Plan, and if so, what approvals are required.
3. FIRE COMPARTMENTALIZATION CONCERN
The original corridor end wall served as a fire separation barrier between the shared corridor and the interior of Unit 204. Its replacement with an approximately 75-inch-wide double
door opening — occupying nearly the entire width of the corridor end wall — may materially affect the fire compartmentalization design of the second floor.
I respectfully request that LA County Fire Department independently verify that the fire separation between the corridor and Unit 204 continues to meet the required fire-resistance rating
following this alteration, and that the double door assembly has been confirmed as a compliant fire-rated assembly with the required closure and locking hardware.
4. REQUEST FOR CONFIRMATION BEFORE FINAL SIGN-OFF
Given that the permanent sealing of Unit 204's original corridor entry door and the relocation of the fire extinguisher cabinet do not appear to have been disclosed or addressed in any
prior inspection or correspondence, I respectfully request that no final inspection sign-off be granted until written responses to the above items have been provided.
I am also attaching an on-site measurement photograph showing the perpendicular clearance between the Unit 204 double door frame and the Unit 203 side wall, measuring approximately 11
inches, being the perpendicular distance from the Unit 204 double door frame to the Unit 203 side wall, for reference in connection with the ADA maneuvering clearance concern raised
in my prior correspondence dated June 24, 2026.

1. FIRE EXTINGUISHER — "TEMPORARILY PLACED ON THE EXISTING DOOR"
With respect, mounting a fire extinguisher on a swinging door does not constitute a compliant installation under California Fire Code Section 906.6, which requires fire extinguishers
to be installed on fixed hangers or in listed brackets at an approved location. A door is neither a fixed hanger nor a listed bracket. It moves, it swings, and it can be blocked. This
is not an acceptable interim or permanent solution.
Furthermore, your response confirms that as of today, no approved permanent fire extinguisher location exists. Inspector Ortiz was previously advised by Building Official Tao that the
extinguisher would be returned to its "original location." Building Official Tao's current response now confirms that the original location no longer physically exists, and no compliant
replacement has been identified.
I formally request:
(a) The specific proposed permanent location of the replacement fire extinguisher, in writing;
(b) Confirmation that the proposed location complies with the 75-foot travel distance requirement under CFC Section 906 for all occupants on the second floor; and
(c) Confirmation that LA County Fire Department has approved or will approve the permanent location prior to final inspection sign-off, as required by City Council Resolution No. 2014-13,
Condition 27.
2. CITY COUNCIL RESOLUTION NO. 2014-13 — CONDITION 25 REMAINS UNADDRESSED
For the third time, I respectfully note that neither response has addressed City Council Resolution No. 2014-13, Building and Safety Condition 25, which states:
"Existing rated corridors shall be maintained and existing penetrations uncovered during inspections will be required to be fire protected."
This is a mandatory condition adopted by the Diamond Bar City Council that runs with the land and binds all current owners. It is a separate and independent legal obligation from California
Building Code compliance. General building inspection sign-off does not satisfy this condition. I am formally requesting written confirmation from the Planning Division — not the Building
and Safety Division — that this work has been reviewed for compliance with Condition 25.
3. ADA MANEUVERING CLEARANCE — ACTUAL MEASUREMENTS REQUIRED
Building Official Tao's response states that "the doors swing in so no encroachment occurs for ADA access." With respect, the direction of door swing is not the only factor under CBC
11B-404.2.4. The required 18-inch latch-side maneuvering clearance at the 90-degree corner adjacent to my Unit 203 entrance must be physically measured and verified on-site. A verbal
assurance regarding door swing direction does not constitute verification of ADA compliance at a corner configuration.
I formally request the actual measured distance between the Unit 204 door frame and my Unit 203 entrance, in writing.
I note that all three of the above issues remain open and unresolved. I respectfully request that final inspection sign-off not be granted until written responses to items (a), (b),
and (c) above have been provided, and until the Planning Division has confirmed compliance with Resolution No. 2014-13 Condition 25.

1. PHYSICAL IMPOSSIBILITY OF FIRE EXTINGUISHER RELOCATION
Inspector Ortiz's email states that "the Fire Extinguisher will be placed back in its original location." With respect, this is physically impossible. The original location was a fully
recessed cabinet embedded within the corridor end wall. That wall has been completely demolished and replaced by Unit 204's double door frame. There is no wall remaining at that location.
I am requesting written clarification of exactly where the fire extinguisher will now be placed, and confirmation that its new location meets the 75-foot travel distance requirement
under California Fire Code Section 906 for all occupants on this floor.
2. CITY COUNCIL RESOLUTION NO. 2014-13 — CONDITION 25 HAS NOT BEEN ADDRESSED
Neither response addresses the City's own Conditions of Approval under City Council Resolution No. 2014-13, which runs with the land and binds all current owners. Building and Safety
Condition 25 of that Resolution explicitly states:
"Existing rated corridors shall be maintained and existing penetrations uncovered during inspections will be required to be fire protected."
General California Building Code compliance is a separate and independent standard from the City Council's Conditions of Approval. A building permit inspection addresses the former,
not the latter. I respectfully request written confirmation that the Planning Division has reviewed this work specifically for compliance with Condition 25 of Resolution No. 2014-13,
and that the Community Development Director has been consulted as required.
3. ADA MANEUVERING CLEARANCE — CBC 11B-404.2.4
Neither response addresses the ADA latch-side maneuvering clearance violation I raised. The new double door frame of Unit 204 is positioned less than 18 inches from the entrance to my
Unit 203 at the 90-degree corridor corner, in direct violation of CBC 11B-404.2.4, which mandates a minimum 18-inch latch-side clearance. I request written confirmation that this specific
measurement has been physically verified on-site.
FORMAL REQUEST FOR JOINT ON-SITE INSPECTION
Given the above, I formally request a joint physical on-site inspection by both the City of Diamond Bar Building and Safety Division and the Los Angeles County Fire Department at 1470
S. Valley Vista Drive, Second Floor Corridor, to physically verify:

Dear Inspector Joe Ortiz and City Building Official,
Thank you for your response. However, the verbal assurances provided by the City Building Department contain factual misrepresentations that severely compromise public life-safety.
1. Physical Impossibility of Extinguisher Relocation: As shown in the attached historical photograph, the mandated public Fire Extinguisher Cabinet was FULLY RECESSED inside the structural
core of the wall. That exact spatial location has now been completely demolished and replaced by Unit 204's private double door. It is physically impossible to place the recessed cabinet
back in its "original location" without removing their unpermitted massive double door. It is highly alarming and illogical that the inspector could assure that the fire extinguisher
will be placed back in its "ORIGINAL LOCATION"—BECAUSE THE WALL IS NOW GONE. How exactly is this physically achievable? Do they plan to construct a heavy fire extinguisher cabinet directly
onto a moving, swinging wood door? Because that door frame is currently where the original location used to be.
2. Violation of City Council Resolution No. 2014-13 (Condition 25): Compliance with the general California Building Code is legally insufficient here. This specific property is strictly
bound by Diamond Bar City Council Resolution No. 2014-13, which runs with the land and binds all successors.
The mandatory condition under the Building & Safety section explicitly mandates: "Existing rated corridors shall be maintained and existing penetrations uncovered during inspections
will be required to be fire protected." Cutting through an established continuous fire barrier to install a private double door is a direct violation of the City Council’s mandatory
administrative resolution.
A copy of the said City Council Resolution No. 2014-13 can be provided immediately upon request.
3. Encroachment of ADA Clearance (CBC 11B): The new door frame sits less than 1 foot from my entrance (Room 203) at this 90-degree corner, permanently violating the mandatory 18-inch
ADA latch-side maneuvering clearance required under CBC 11B-404.2.4, creating an illegal pinch-point hazard for any individuals entering my property.
Please accept this email as formal administrative notice that the current modification is profoundly non-compliant. I request that the Los Angeles County Fire Department and a City Supervisor
conduct an actual physical on-site inspection of these violations, rather than relying on unverified verbal reports, to avoid liability for signing off on a documented fire and ADA
hazard.

My name is Inspector Joe Ortiz with the Los Angeles County Fire Department. I have spoken with the City of Diamond Bar Building and Safety Official regarding the TI work that was recently
done at 1470 Valley Vista Drive, Unit 204. The inspector has confirmed that this was all done with permits and abides by the California Building Code. He has also assured me that the
Fire Extinguisher will be placed back in it’s original location. The double doors leading to unit 204 are Fire Rated which is up to Code. The inspector has also assured my that the
path of egress has not been obstructed by the modifications done to unit 204.

Dear LA County Fire Prevention Marshal,
I am the owner of Unit 203, the direct neighbor of Unit 204. Instructed by the Diamond Bar Building Safety Department, I am formally reporting an active, unpermitted structural violation
and severe fire hazard that directly threatens my life safety and blocks my primary emergency exit route.
The occupant of Unit 204 has unlawfully cut open a fire-rated common area corridor firewall to install a massive double door that is significantly higher and deliberately leaning heavily
toward my unit's entrance, cutting deep into the upper section of this solid common area firewall to artificially elevate the door height. This poses an immediate, catastrophic risk
to the fire separation of the entire floor. This illegal alteration is located extremely close to my own entrance door, directly disrupting my only emergency escape path (means of egress)
in a narrow hallway. Furthermore, they have unlawfully removed the building's common area Fire Extinguisher cabinet on this wall prior to receiving official approval. Property management
is failing to stop this dangerous construction.
According to Page 36 (Sheet A-701) of the Original Building Blueprint, the corridor wall they demolished is explicitly designated as UL DESIGN NO. U465 (1-Hour Fire-Rated Interior Partition
Assembly). The tenant completely breached this certified fire safety envelope without any approved Fire Plan Check, stripping the corridor of its mandatory smoke and fire isolation
protection. I tried to upload the said physical page but failed due to file size limitations; I have it fully saved and documented on my end, and I am ready to forward it directly to
your email or hand it over upon request.
1. Breach of Certified Fire Barrier: The tenant completely breached this certified fire safety envelope without any approved Fire Plan Check, stripping the corridor of its mandatory
smoke and fire isolation protection.
2. Severe Psychological Distress & Insomnia for Record: This ongoing illegal construction and the complete removal of the hallway fire extinguisher have caused me severe psychological
distress, severe anxiety, and continuous clinical insomnia. I have been completely unable to sleep for days out of constant panic and terror that my only emergency fire egress path
(Sheet A-002, Path of Travel) will be physically trapped and blocked by their massive non-compliant double door when opened outward.
3. Unpermitted Scope Encroachment: The blueprint confirms this is a Non-Bearing Corridor Common Area Wall. The tenant’s active permit (PR20250001558) is strictly limited to internal
TI framing. They have zero legal right to break out into the common walkway or alter the structural partition. According to our CC&Rs, even if they have the City's permit, they still
strictly need the Master Association's approval to alter any common area.
4. Willful Concealment of Approvals: In May 2026, the exact property manager issued an official Violation Notice to Unit 204 via email for drilling unauthorized holes (before they knocked
off the wall and removed the fire extinguisher cabinet) into this common corridor wall, demanding them to restore it. Yet on June 15, 2026, the same manager completely backtracked,
claiming this illegal double-door construction is "fully approved." This history proves that both the tenant and management had prior, full knowledge that this structural wall is a
restricted common area, yet they chose to bypass codes to create a fait accompli. They are operating in gross bad faith. I can show you the email if needed.
Also on June 15, 2026 the property management admitted that "What we did manage was the request for the unit owner to have a licensed vendor oversee the relocation of the Fire Extinguisher,
which must be approved by Property Management before installation. This part of the project is still pending completion".
I have formally requested the property management company to provide the mandatory City and Master Association approval documents for this common area demolition. However, they have
flatly refused to disclose any records. The property management is actively concealing this hazard and bypassing the Master Association, likely keeping the actual property owner completely
in the dark, because no recorded landlord was included in the Violation Notice email that they sent in May 2026 (I'm not sure if DIGITALSYSTEM TECHNOLOGY INC is the landlord, but they
are not the landlord on record as my best knowledge).
An inspection from CITY was conducted on Tuesday, June 16th 2026, however, there was no one on site so no access was gained; correction notice was issued. They have reached out to the
contractor and have scheduled an inspection for Tuesday, June 23rd to look further into this matter.
I am attaching the official video screenshot included in the Violation Notice in May 2026 that property management sent to Unit 204 before the corridor firewall was demolished; and a
picture of the illegally installed massive double door, and the correction notice from City. The video is too large to send via email, I can show you the Violation Notice email/video
