Binding Transmittal Letter Proposer must provide a cover letter (two pages maximum) signed by a party authorized to contractually obligate the proposer (and respective team members) to perform the commitments included in the proposal. The letter must also identify the name, title, address, telephone number, and e-mail address of the individual(s) responsible for such negotiations, if selected. Additionally, the cover letter should discuss the proposer’s overall qualifications and ability to meet the scope of services outlined in the RFP, name the individual proposed to serve as Construction Manager, and include a statement guaranteeing the validity of the Cost Proposal for a period of 120 days beyond the bid opening. The letter shall also constitute certification, under penalty of perjury, that the Consultant will meet all City rules and regulations, and State and Federal nondiscrimination laws. Technical Proposal The Technical Proposal shall include the following components: i.Introduction In addition to the name, mailing address, telephone number of the firm, and contact information for the firm’s authorized contact person (telephone and email address), provide an overview of the proposal demonstrating the understanding of the project and the objectives for completing the tasks identified in the scope of work. ii.Qualifications Provide a detailed account of the firm’s experience and qualifications, including: A description of the firm’s capabilities and experience managing similar projects. A description of the capabilities and experience of the proposed Project Manager and all on-site support staff, including brief resumes of each that highlight qualifications and abilities relevant to the project. An organizational chart identifying all key personnel, their specific project responsibilities, and the relationships/chain of command between staff and the Project Manager. Identification of any subcontractors the proposer intends to employ in the execution of the project, including name, address, telephone number, and name of contact person. Discuss the proposed subcontractor’s role and provide information on subcontractors’ experience performing similar work. All subcontractors are subject to the approval of the City. The proposed new devices would be installed in the same location as the current devices, subject to relocation as requested later by the City. All products and services will be provided as the responsibility of Pacific Office Automation with installation services provided by a subcontractor of POA. Scope of Services Proposal must provide detailed information that clearly demonstrates the ability of the proposer to fully deliver the scope of services outlined in the RFP The following is a broad outline and not exhaustive Transition Plan for the City of Diamond Bar. This guiding plan may be tailored per your request, but the general approach to managing a vendor change has been consistently successful when we have implemented similar solutions and deliverables within organizations. POA will work with you to understand and meet whatever time constraints, requirements and expectations may exist due to current contract restrictions or any potential resource conflicts due to any other parallel initiatives. This is a further discussion we would like to have so we can understand how to best meet the needs of the City of Diamond Bar with as minimal disruption as possible. Introductions / Expectations Meeting This is a strategic meeting to establish and review dates for each location and set priorities to specific locations/departments for key elements of the implementation. Initial implementation schedule and framework developed between POA and the City. A designated Point of Contact (Kasey Kunichika) shall be assigned to the City account for implementation and project management. Meet with the City IT team Meet with the City Administrative team Review contractual requirements and timelines Review resource allocations needed to meet requested timeline Walk-Throughs, Mapping and Site Coordination Implementation phase to review and have consensus on workflows expectations for each site location. POA shall gather and document detailed technical systems requirements for the system and deliver a requirements validation report. Prepare the City site plans to plan the design and configuration of a proposed site. Original plans shared and collaborated with the City key stakeholders o Make recommendations for optimal amount of security cameras o Address future planning and standardization System Configuration A properly built network is critical for the successful implementation of any modern camera system including Verkada. Prior to installation of the new camera system, a site survey will be performed which will identify the type of network switches, number of ports available, possible camera placement location, etc. In the event of non-complying equipment, we will work with the customer to resolve the issue prior to implementing the new camera system. Demo security cameras ordered to first test a standardized configuration in a controlled environment o Internal testing of system Review of demo testing outcomes o Adjustments/changes to configurations addressed o (Change orders placed to correct any issues) 10 Installation and System Implementation Once the City agrees to site plan with the design and configuration, we would then deploy the cameras and move on to implementation. Purchase orders are placed by the City and cameras are shipped to site locations per agreed upon schedules. o Installing electrical company confirmed for installation Cameras are installed and tested for mounting angle and point of view Full access and functionality of installed security camera(s) provided to the City o The City to provide feedback on POV and implementation while on-site Maintenance and Operation Services Pacific Office Automation will provide the cameras and Verkada provides a 10-year warranty on the camera itself. Support and maintenance services shall include, at a minimum: Application management, help desk support, enhancements, maintenance, and documentation updates. Firmware updates. Performance monitoring, measuring, and reporting. Business Continuity and Disaster Recovery Service. Enterprise Technical Support (phone, email and live chat) is included as part of your Verkada license at no additional cost. Support hours: Monday through Friday, 8am to 5pm (U.S. Pacific Time) 24/7 support available on request 4. Conflict of Interest The Consultant shall disclose any financial, business or other relationships with the City and County that may have an impact on the outcome of this contract or any resulting construction project. The Consultant shall also list current clients who may have a financial interest in the outcome of this contract. Pacific Office Automation, Inc. or staff members currently have no relationship with the City of Diamond Bar or the County that would impact either the evaluation of our proposal or outcome of the RFP. Pacific Office Automation provides this proposal free of Conflict of Interest. 5. Litigation Indicate whether the proposer or any team member or any officers or principals have been party to any lawsuit involving the performance of construction management duties and provide a summary of the issues and status of the lawsuits. Pacific Office Automation is not a defendant in any litigation as a result of failure to perform duties of a contract. Any current litigation is for the purpose of collection of payment of services from delinquent accounts. 11 4.
