Mr. Di Bias will be the main point of contact for the duration of the project and will ensure that all aspects of the project are completed in accordance with the requirements and schedule as set forth by the City. PROFESSIONAL EXPERIENCE Mr. Di Bias has over 30 years of experience with Truxaw and Associates, being involved in all aspects of the civil engineering and land surveying services provided by the company. In his role, Mr. Di Bias is responsible for directing the administration of the firm as well as management of the civil engineering and land surveying operations performed by the Truxaw team. Mr. Di Bias specializes in civil engineering, water quality design, development of cost-effective design solutions, quality control, constructability oversight, and project management. He also directs the engineering design on numerous company projects. He is also responsible for oversight and management of the survey department and many survey projects performed by the company. Mr. Di Bias works closely with the company’s Professional Land Surveyors to ensure that the client’s expectations are being met and that effective communication is provided. Truxaw and Associates, Inc. is prepared and qualified to provide professional land surveying services to the City of Diamond Bar. Truxaw and Associates has proven experience providing professional civil engineering and land surveying services for both the private and public sectors. Our track record proves that we have the necessary knowledge, resources and experience combined with effective management capabilities to successfully support the City’s project needs. The following steps will be performed during the course of the survey of the Pony and Sunset Crossing properties: Boundary / Property Line Survey a) Truxaw has thoroughly reviewed the project description, and when awarded the project we will conduct public record research as required. This proposal assumes that the City will provide a title report covering both the Pony Property and the Sunset Crossing Property at the time the project is awarded. b) The Boundary Survey will be prepared in accordance with applicable State law, local customs and the City of Diamond Bar requirements. Truxaw’s recent and relevant experience often expedites control work necessary to complete a boundary task. c) The survey data will be analyzed, and the project boundaries will be established based on found survey monuments of record and substantiated by lines of occupation as field surveyed. d) Once the property boundaries have been established, Truxaw will set permanent boundary monuments at all property corners and at 100-foot intervals along the easterly property line of the Pony Property adjacent to the homes along Big Falls Drive. e) Once the permanent monuments have been set, Truxaw will prepare a Record of Survey to document the setting of these monuments in accordance with the requirements of Section 8771 of the California Business and Professions Code. Truxaw will also file the Record of Survey with the office of the County Surveyor. f) Truxaw will provide the city with digital copies of the Boundary Survey in both .pdf and GIS shapefile/layer formats. Electronic AutoCAD files, ASCII coordinate point files of the completed project survey, and RAW field book data files will also be maintained and available to the city, if requested. Utility Survey a) Once the project has been awarded, Truxaw will request copies of atlas maps and record plans for sewer, water and storm drain facilities within the project area from the City. Truxaw will also request copies of substructure maps from utility companies understood to have facilities within the project area. This proposal assumes that the City will provide contact information for these utility companies. b) Truxaw will plot surface utility features such as utility poles, manholes, water valves, meter boxes, pull boxes, and other visible features based on field survey data. c) Truxaw will retain C Below to locate existing underground utility lines by using methods such as electromagnetic locators, ground-penetrating radar, etc. d) C Below will prepare a detailed utility survey map that will indicate the locations of water, sewer, gas, electricity and telecommunications lines. This map will include both above-ground and underground utilities, including valve locations, and include a legend to identify the various utility types. The location and approximate depth to each utility will be noted when able to be determined. This map will also include utilities within the sidewalk areas adjacent to the Pony and Sunset Crossing properties. e) Digital copies of the Utility Survey will be provided in both .pdf and GIS shapefile/layer formats. Electronic AutoCAD files will also be made available to the city, if requested. Truxaw and Associates, Inc. Truxaw further understands that said coverage is a prerequisite for entering into an agreement with the City of Diamond Bar. Consulting Services Agreement Jospeh C. Truxaw and Associates, Inc. agrees to the terms and conditions of the City’s Consulting Services Agreement with the following exception: • On page 5 Item B, the words “agents and volunteers” cannot be named as additional insureds on the certificate of insurance.
