B. Project Team

Approach and Methods to Meet the Scope of Work
1. Public Opinion Survey
a. Review and Analysis:
• Approach: Conduct a thorough review of the City’s culture, current budget, Capital
Improvement Program, and infrastructure needs.
• Methods: Utilize existing city documents, financial reports, and strategic plans to
understand the context and requirements.
b. Revenue Measure Options:
• Approach: Identify and evaluate potential revenue measure options.
• Methods: Analyze historical data, benchmark against similar municipalities, and consult
with financial experts to compile a list of viable options.
c. Election Cycle Analysis:
• Approach: Recommend the best election cycle for a potential revenue ballot measure.
• Methods: Assess voter engagement trends, historical election data, and community
priorities to determine optimal timing.
d. Survey Design:
• Approach: Design a comprehensive survey questionnaire.
• Methods: Develop questions in English, Chinese (Mandarin), and Korean. Ensure
statistically valid sampling methods to achieve a 95% confidence level with +/- 5%
accuracy.

E. Cost for Services

the hourly allocation of resources by discipline and by each task identified in the Scope of
Work, include billing rates for personnel, printing, attendance at public meetings, and any
other cost anticipated. Please clearly highlight any additional or recommended tasks and
include associated costs by listing them as “Optional.” No additional charges for items such as
transportation, out-of-pocket expenses, etc., will be paid unless these are specifically included
in the proposal.

This detailed cost proposal ensures that all aspects of the project are covered, and the City of
Diamond Bar can have a clear understanding of the financial commitment required.
F. Insurance
Proof of insurance requirements addressed in the professional services agreement of this
Request for Proposal shall be submitted by the selected Consultant upon execution of the
contract as defined under the City’s existing purchasing Ordinance. The selected Consultant
must submit a "Statement Certifying Insurance Coverage" certifying that the required
insurance coverage will be obtained by the Consultant, and that the Consultant understands
said coverage is prerequisite for entering into an agreement with the City. The Consultant is
required to confirm with its insurance carrier that it can meet all the requirements for
insurance. Failure to meet the insurance regulations as set forth shall result in proposer’s
disqualification.
Leap Agency, LLC certifies it will provide necessary proof of insurance requirements
addressed in the professional services agreement, and certifies an understanding that the
aforementioned requirements are a prerequisite for entering into an agreement with the City.
G. Consulting Services Agreement
Provide a statement certifying that you agree to the City’s Consulting Services Agreement
terms and conditions. Any proposed edits to the agreement shall be submitted with the
proposal for staff’s review and consideration.
We hereby certify that we agree to the City of Diamond Bar’s Consulting Services Agreement
terms and conditions as outlined in the Request for Proposals for Public Opinion Polling
Services. We understand and accept all the terms and conditions specified and are prepared to
comply with them fully upon entering into an agreement with the City.
