At a previous Area D meeting, I mentioned form 214. I added a CITY EOC option on the dropdown of the form. We used this at the County EOC, which simplified our response, considering
our position went virtual and remote.
You can use this for documentation of your COVID-19 EOC activation. You do not need to send this to anyone, but retain for documentation purposes in the event you are ever audit for
this incident.
