I need to add Unit G12 to my permit (Unit G12 has been done with inspection but did not show on permit.

Iām the project coordinator and am working remotely the remainder of the week. Due to this, Iād prefer the information be emailed to me but itās not required. The contractor may drop
off a check for $ 774.25 with a copy of this email for reference along with the other requested information, Mon-Thur, 7:30am-5pm, Fri, 7:30am-4pm. The staff at the office will then
need to scan the information and email it to me directly. I will then need to prepare the permit document and have the contractor sign it.
Perhaps it would be best for me to coordinate directly with the contractor when Iām in the office and have him come in then. Please provide me with his contact information so I can
give him a call. I want to avoid further delays.

Per your request and our phone conversation earlier today, the total permit fee for the balcony repairs & temporary shoring is $ 774.25. The contractor may be subject to a new Business
License ($ 42.82) or a renewal ($14.00).
Also, I will need the information below to start preparing for permit issuance. Please also forward this email to the contractor responsible for obtaining the permit.
* Completed Business License form from the contractor (see attachment)
* Copy of the contractorās license
* Copy of the workerās compensation certificate
* Contractorās full name, phone number and email address. They will need to digitally sign the permit
When do you anticipate submitting the payment and having the contractor obtain the permit?
