I’m the project coordinator and am working remotely the remainder of the week. Due to this, I’d prefer the information be emailed to me but it’s not required. The contractor may drop
off a check for $ 774.25 with a copy of this email for reference along with the other requested information, Mon-Thur, 7:30am-5pm, Fri, 7:30am-4pm. The staff at the office will then
need to scan the information and email it to me directly. I will then need to prepare the permit document and have the contractor sign it.
Perhaps it would be best for me to coordinate directly with the contractor when I’m in the office and have him come in then. Please provide me with his contact information so I can
give him a call. I want to avoid further delays.

License ($ 42.82) or a renewal ($14.00).

* Copy of the worker’s compensation certificate
