We did a thorough structural inspection last week. The files are being compiled with structural deficiencies identified. We will go over it with the client mid this week and prepare
the repair detail accordingly. Still shooting for submitting the package around the end of this month.

The plans have returned approved. I will need the information below to start preparing for permit issuance. Please also forward this email to the contractor responsible for obtaining
the permit or any other applicable party.
* Please provide the (6) unit numbers impacted by this scope of work
* Completed Business License form from the contractor (see attachment)
* Copy of the contractor’s license
* Copy of the worker’s compensation certificate
* Contractor’s full name, phone number and email address. They will need to digitally sign the permit
* Preferred payment method (Checks can be made out to the City of Diamond Bar and either mailed to the address below to my attention. Or made with a card which is subject to a 2.5%
convenience fee)
* Payor information (full name, email address & phone number) – ONLY IF PAYING WITH A CARD
