Route 66, #431___________________ _Glendora, CA 91740______________________ NOTICE TO PROCEED DATE: __10.17.22__SCHEDULED COMPLETION DATE: _1.25.23___ ORIGINAL CONTRACT AMOUNT:$___674,377.50____________ CONTINGENCY AMOUNT AUTHORIZED BY COUNCIL:$_101,156.00___ TOTAL CONTRACT AUTHORIZATION:$_775,533.50_________________ TOTAL AMOUNT OF CONTRACT PRIOR TO THIS CONTRACT CHANGE ORDER: $_707,733.00 CONTRACTOR IS AUTHORIZED TO MAKE THE FOLLOWING CHANGES: REMOVED FROM SCOPE: 1.Removal of all 3rd tier cages on Gabion Wall #3, as the grades behind this Gabion Wall do not warrant the additional height, this will delete the installation of (41) 12” high cages, and (11) 24” high cages. Credit to the City for labor costs of $1,959.72. 2.Please see Location #4 on attached plan (11x17 version) / Westerly-End of Trail Improvements A. Delete the Concrete Swale as indicated on the approved Plans that wraps around the backside of the Oak Tree grove, as indicated on the Approved Plans / 71 LF B. Delete the Velocity Reducer as indicated on the approved Plans for the above-described location / 5 SF ADD TO SCOPE: 1.Add three Gabion cages to east end Gabion Wall #1 as discussed on 12.19.22 during site walk. Install Concrete Swale directly adjacent to the north side of the timber step improvements, running the full length of the timber steps complex / Estimated at 80 LF. ($92.43 per L.F.) B. Customize the design of the Velocity Reducer at the extreme west end of the trail improvement project / timber step complex as follows: -Start at Concrete Swale’s low end @ 4’ wide, widened to 14’ wide, and is to be 10’ long / 90 SF. Cost to the City (102sq.ft x $420.00) $42,840 -Quarry Stone Rubble pressed deeply into a 12” deep concrete base slab (See Detail #5 on Title Sheet of approved Plans). 3.Add “Splash Wall” at the west end of Gabion Wall #1 concrete swale; installing GROUTED rip-rap (in a similar manner as velocity reducer detail), from end of concrete swale to edge of trail / and trail water diverter, with timber-tie edges (per current contracted improvements). Slope rip rap towards end of concrete swale to act as a splash wall.
